Supplier packaging environmental compliance form

This form is to be used by all packaging suppliers to affirm compliance to various environmental regulations including heavy metals content and other related items. It is based on the EPEAT criteria (Electronics Packaging Environmental Assessment Tool) which is an industry and EPA (U.S. Environmental Protection Agency) sponsored program as well as compliance to mandatory EU Packaging Directives (94/62/EC).
